A document used to give an "agent", also known as the "attorney in fact", the legal power to make decisions on your behalf.  The extent of these powers is up to the person signing the document. Many people decide it is best to give a spouse, child, or other trusted person a significant degree of latitude to act in their stead; particularly in the event they are unable to make decisions for themselves.
